Accepted Insurance
"Accepted" means that Family HealthCare Associates has agreed to provide care to the customers of a particular insurance program. We would like to be able to participate in all insurance programs; however, this is a complex decision which considers the availability of hospitals, specialists, referral restrictions, drug formularies and reimbursement.
Please bring your insurance card to every visit!
Please note: Specific benefits and copays are determined by your plan. If we participate in your health plan, we will file a claim with your insurance carrier for the services provided. However, a co-payment or deductible may be due at the time of service.
If we do not participate with your health plan, full payment for your services will be collected at the time of your visit. As a courtesy,for most insurance companies we will file the claim to your insurance carrier and they will reimburse you directly. Please see our Payment Policy for additional details.
We are pleased to participate in Secure Horizons HMO, a Medicare Advantage Plan. This program will soon be designated as AARP Medicare Complete through Secure Horizons. Please note that we do not at this time file on Medicare.
