The following is important news from your physicians at Family HealthCare Associates. We want to make sure to inform you about an upcoming change here at FHCA … and what it DOES and DOES NOT mean to all our valued patients.
Network Change
FHCA will be changing our affiliation from Southwestern Health Resources (SWHR) to Catalyst Health Network (CHN) effective January 1, 2022.
Patient Impact
Because of this network switch, you may receive a letter from your insurance company (UnitedHealthcare, BlueCross BlueShield, Cigna, Aetna, as well as others not listed) during the transition period stating FHCA is now out of network. This is not true. FHCA will remain in-network for our patients by FHCA’s participation in Catalyst Health Network. You will still be able to access FHCA as an in-network primary care provider.
No action is required on your part. As always, thank you for trusting FHCA as your healthcare provider in all of life’s moments.
