Family HealthCare Associates is pleased to announce our participation in the Together 2 Goal campaign. Together 2 Goal is a three-year initiative that challenges leading healthcare organizations to work together to transform diabetes care for millions across the country. We strongly believe that over the next three years, with dedication and commitment, we will see improved care among people with Type 2 diabetes and set them on a better path to live longer, healthier lives.

Type 2 Diabetes

Diabetes is a problem with your body that causes blood glucose (sugar) levels to rise higher than normal. This is also called hyperglycemia. Type 2 diabetes is the most common form of diabetes. If you have Type 2 diabetes your body does not use insulin properly. This is called insulin resistance. At first, your pancreas makes extra insulin to make up for it. But, over time it isn’t able to keep up and can’t make enough insulin to keep your blood glucose at normal levels. http://www.diabetes.org/

Type 2 diabetes is a serious condition that if not controlled can cause vision loss, kidney damage, nerve damage, foot ulcers, heart disease, and possible amputation from infections.

About the Together 2 Goal® Campaign

The AMGA Foundation, the nonprofit arm of the AMGA, launched Together 2 Goal®, a national campaign to fight the challenge of Type 2 diabetes on March 12, 2016. Together 2 Goal® includes over 100 medical groups, health systems, non-profit partners, and corporate collaborators such as the American Diabetes Association, American Association of Diabetes Educators, Novo Nordisk, Inc., Geisinger Health System, Henry Ford Health System, Intermountain Healthcare, and Ochsner Health System. The goal of the three-year campaign is to improve care for 1 million people with Type 2 diabetes by 2019. www.Together2Goal.org
